Position Summary:

 

This position is responsible for the oversight and data entry for the Stroke Program including the integrity and accuracy of information produced within the Program.  This person facilitates stroke rounds, quality outcomes, and process management of the indicators for the Program.  Monitors compliance of nursing staff and Physicians within established indicators and regulatory criteria; conducts follow-up on variances.  Collaborates with internal and external resources to ensure effective planning, implementation, and evaluation of the stroke program. Mentors and coach’s staff and physicians to expand knowledge of stroke and limit variances in performance.  Collaborates with team members throughout the medical center to ensure effective survey readiness. This person is responsible for ensuring consistency in timely reporting, follow up and explanations of variances.  As an educator this person facilitates the enhancement of nurse cognitive skills and competency for the care of stroke patients. The job encompasses integration, facilitation and coordination of the leadership, operational, and clinical activities of stroke services. The individual must consistently support the mission, vision and values of the Medical Center, and acts as a role model relative to the Medical Center’s Service Standards.

 

 

Major Position Responsibilities:

 

  •  Responsible for overseeing all stroke quality indicators on a regular basis.
  • Ensures the maintenance of appropriate linkages to Quality committees within the hospital.
  • Enters data in Get with the Guidelines, Joint Commission and other regulatory data banks as requested.
  • Ensures distribution to post discharge patients and analyzes customer satisfaction responses from Press Ganey patient satisfaction tool ongoing basis.
  • Assures orientation and appropriate training programs are developed and maintained in areas of responsibility.  Promotes professional growth of the staff through educational programs, committee and/or task for involvement.
  • Follows budget and financial guidelines.
  • Assures that the unit members have standards that standardize stroke care, admission/discharge criteria that meet stroke criteria.
  • Assures staff is clear on job expectations and performance measures as they relate to stroke care.
  • Provide input into staff evaluations.
  • Develops annual goals and objectives that support the stroke program and related business plans developed by the service line committees.
  • Complies with all regulatory requirements and GWTG requirements to maintain program eligibility.
  • Prepares for all surveys with a comprehensive plan. Coordinates all activities, provides direction and clarity for meeting criteria.
  • Participate, as requested, in integration efforts with other stroke initiatives in Southern California.
  • Enters data, analyzes date and makes recommendations to the Stroke Team.
  • Demonstrates awareness of self-learning needs and seeks ways to meet these in order to maintain current competency and respond to new leadership demands.
  • Demonstrates ability to change through implementation of creative programs that support achievement of strategic initiatives of the Medical Center.

 

Qualifications:  

  • Minimum of three (3) years nursing experience in a related field (minimally inpatient Med/Surg nursing) required. 
  • RN on stroke unit, preferred. 
    • Certification in administration by any nationally recognized nursing or clinical organization (e.g. RNC, ANCC, CCNR), preferred.
    • Experience with product line development preferred.
      • At least 3 years of nursing experience required in related field. 
      • Must also have the ability to train and develop other personnel by providing them with the knowledge gained through years of experience on the job, required.
      • Knowledge of the business of health care along the continuum of care, (patient admission through discharge to step down rehab care), required.
      • Clinical decision support data analysis/synthesis, and quality improvement and regulatory knowledge, preferred. 
      • Understands physiological principles involved in the care of the critically ill and acute care patients.
      • 5 years or greater of management experience required in related field, preferred

 

Education:  

  • Graduate of an accredited school of nursing, required.
  • Graduate of an accredited school of nursing Bachelors Degree (BSN), preferred.

 

Licenses:

 

  • Current California RN License, required.

 

Certifications:

  • BLS, required.
  • ACLS, preferred.
  • NIH, preferred.

 

Each new hire candidate who is offered employment must pass a physical evaluation, urine drug screen and pre-employment background checks before starting work.
