Position Summary:

Provides safe, accurate and cost-effective pharmaceutical care to the patients of TCMC. Provides leadership and coordinates staff development opportunities. Responsible for the development of clinical pharmacy programs.  Responsible for Quality Assurance activities relating to drug usage and therapy within the hospital.  Responsible for developing clinically-oriented educational programs for pharmacist staff development.  Must be able to develop positive relationships with physicians on the Medical Staff and serve as a liaison for Nursing. Other activities will include monitoring drug usage criteria, physician intervention when appropriate, and assessing patient outcomes based on intervention activities. Evaluates staff pharmacists’ clinical abilities and performance on a yearly basis and will provide input to the Pharmacy Operations Manager for annual evaluation. In the absence of the Director of Pharmacy and Pharmacy Operations Manager assumes responsibility for the operation of the Pharmacy.

 

Major Position Responsibilities:

  • Responsible for the patient-oriented pharmacy practice.
  • Responsible for the direct care to patients in the following age groups:
    • neonatal
    • pediatric
    • adolescent
    • adults
    • geriatrics
  • Development/Modifications of clinical pharmacy programs.  These will include, but not limited to a pharmacokinetic program, anticoagulant therapy program, renal insufficiency dosing program, Coumadin dosing program, anticoagulation program, intravenous to oral conversion program, etc
  • Evaluates, develops and implements educational programs for pharmacists.
  • Responsible for Quality Assurance activities relating to drug utilization throughout the hospital.
  • Maintains an active role on the Pharmacy and Therapeutics Committee. Responsible for preparing and presenting clinical information to the Pharmacy and Therapeutics Committee.
  • The Director of Pharmacy will assign the Clinical Coordinator to various committees of the hospital.
  • Develops, implements and evaluates treatment guidelines that address cost effective and appropriate drug therapy.
  • Must demonstrate a high-level of drug therapy management and knowledge with respect to pathophysiology, drug information, drug interactions, etc., and applies this knowledge to daily practice in a defined patient population.
  • Responsible for maintaining drug formulary.
  • Demonstrates understanding of formulary process.  Identifies and recommends therapeutic alternatives based on defined criteria.  Must intervene in situations where a therapeutic alternative requires an advanced understanding of complex therapeutic issues.
  • Staffs clinical pharmacist shifts occasionally, as needed
  • Will provide input to the Pharmacy Operations Manager on each clinical pharmacist’s clinical abilities, strengths, weaknesses and areas in need of improvement during yearly evaluations.
  • Must be able to develop, promote, and maintain open lines of communication with members of the medical staff.

  • Safety and Infection Control Responsibilities
    • Responsible to maintain a safe and clean work environment, including unit based specific safety and infection control requirements.

Qualifications:

  • Minimum one (1) year critical care and/or emergency medicine pharmacy experience, required.
  • Three (3) years clinical hospital pharmacy experience, required. May consider board certification in lieu of clinical hospital pharmacy experience.

Education:

  • Pharm D, required.
  • Post Graduate Year 1 (PGY1) residency, required.

Licenses:

  • Current license by the California State Board of Pharmacy, required.

 

Certification:

  • ACLS or PhART (Pharmacy Advanced Resuscitation Training), required within 90 days of hire.  

Each new hire candidate who is offered employment must pass a physical evaluation, urine drug screen and pre-employment background checks before starting work. 

 

*Salary/Hourly wage range for this position is posted.  Actual pay will be determined based on verified experience as well as internal equity.
