The COPD Educator provides comprehensive education to patients diagnosed with chronic obstructive pulmonary disease (COPD). Assists Operations Manager and coordinates with Pulmonary Educator in ensuring quality patient care through professional staff development. This position assists in the planning, development and implementation of COPD programs that coordinate high quality, cost-effective care for these patient populations. Collaborates with managers, physicians and patient care staff to identify and resolve system issues. Directly facilitates care functions which expedite the patients’ work-up and follow-up in the hospital as well as in post-acute settings. The Pulmonary Care Coordinator will interface with Skilled Nursing Facility & Home Health Agency staff to implement & evaluate staff & patient education and guidance in effective management of the COPD population.
Major Position Responsibilities:
Leads the Medical Center’s multidisciplinary COPD Task-Force and will coordinate and evaluate projects related to the work of the Task-Force.
Develops implements, coordinates and evaluates employee education programs for patients with COPD across the continuum to include post-acute care settings such as SNF, and home health.
Provides in-hospital respiratory consulting to recommend an education plan prior to discharge of a patient to home health, nursing home or the outpatient environment.
Acts as a resource to the health care team regarding patient education, occurrence reporting and quality assessment.
Develops smoking cessation program for the community promoting and developing awareness and direction for those at risk for increased pulmonary related illness and disease.
Responsible for Quality data analysis reporting as related to COPD to include tracking and trending of patient care processes to identify areas for improvement & to develop confidential corrective action plans
Responsible for improving Facility’s COPD outcome measures that include readmission and mortality rates
Works with Director and Operations Manager as part of the Respiratory Services leadership team.

6 months pulmonary rehab experience required
Minimum of 3 years clinical experience in acute care required
