Certifications: - Current BLS required upon hire; American Heart Association Healthcare Provider. In times of high activity, will assist with credentialing as needed. - Provide staff and users with assistance solving computer related problems, such as malfunctions and program problems. - Test, maintain, and monitor computer programs and systems, including coordinating the installation of computer programs and systems. - Serve as project leader for particular systems projects. - Facilitate medical staff meetings to include generating agendas and minutes with appropriate follow-up. - Facilitate the approval process/implementation of all medical staff governance documents. - Manage, implement and assure compliance with accreditation of the medical staff continuing medical education program. - Work with various service areas regarding their compliance with IMQ standards to ensure correct processes are utilized to ensure compliance. - Review and analyze data within the system and ability to computer printouts and performance indicators to locate problems, and correct errors. - Evaluate and recommend new equipment or software products needed to automate processes. - Develop & Prepare reports from the database i.e, to pre-populate physician applications, and obtain supporting documentation expirations. Preparation of physician demographic changes and terminations and forwarding to all applicable entities. - Provide oversight of special projects as assigned keeping Medical Staff Leadership informed of status of work efforts through weekly reports, project status reports and other reporting as needed. - Development & Maintenance of Medical Staff Webpage, assist with ER Call Schedule issues and offer assistance after hours. - Automation of office practices. - Ability to monitor and effectively manage database and all functions pertaining to web crawlers and overall system maintenance. - Ensures monthly compliance with Federal databases (OIG and GSA monitoring) for all providers. - Develop process to assure Medical Staff/Hospital compliance with OPPE requirement, reflective of 3-8 month cycles of data every 2 years for inclusion in the reappointment of every provider/member (630) of the Medical Staff. - Ability to work with office management to implement new efficiencies and streamline processes - Have advanced knowledge of all Microsoft Products - Have advanced knowledge of running reports from various databases - Ability to review/map current processes and work with Medical Staff Leadership to develop new Lean processes to obtain desired results; - Participation in the development and implementation of technology projects, system implementations and upgrades, and change initiatives tied to the growth and development within the department. - Perform other duties as assigned.   Qualifications: - Minimum of seven (7) years previous related experience in database management with a minimum of five (5) years in the field of medical staff services in an acute healthcare setting. - Knowledge of full range of Medical staff related functions/responsibilities in the acute care setting. - Proficiency in all modules of Microsoft Office, including Word, Outlook, Excel, Access, Publisher and Visio, proficiency in query tools and relational databases and basic statistical skills and training; - Proven ability to initiate, communicate and implement plans and ideas to improve overall system performance. - Highly developed interpersonal and communication skills (both written and verbal) to interact thoughtfully and effectively with others. - In-depth knowledge of accreditation standards and regulatory requirements. - Strong interpersonal skills – builds strong relationships with diverse groups, encouraging accomplishment of mutually established goals - Ability to manage multiple ongoing projects concurrently; strong organizational capabilities. - Knowledge of MD Staff software preferred. They should not be considered as a detailed description of all the work requirements of the position. The characteristics of the position and standards of performance may be changed by TCMC with or without prior notice based on the needs of the organization. - Maintains a safe, clean working environment, including unit based safety and infection control requirements. - Required to cover 24-hour call coverage on an as determined schedule. - Provides and implements discharge planning services to a varied population including high-risk patients. - Performs therapeutic interventions in assisting patients and families in accepting and adjusting to their illness and course of treatment. - Responsible for mandated reporting laws which include but may not be limited to APS, CPS, Domestic Violence, Violent Injury, and Tarasoff reporting. - Responsible for CCS identification, assessments, and applications. - Provides crisis intervention and supportive counseling during hospitalization. - Works closely with Case Managers and outside agencies with discharge planning. - Assesses and utilizes appropriate community resources to transition patient to the community. - Provides education to the patient/family regarding: - Impact of psychosocial stressors on health and recovery and how to decrease negative impact and increase positive outcomes. - Utilization of support systems and community resources to maximize and/or support positive therapeutic outcomes. - Provides education to staff, both formally and informally regarding psychosocial issues as they relate to patient outcome as well as community resources to assist with post discharge care. - Communicates clinical assessment information appropriately via progress notes, verbal exchanges with nursing, physicians and other healthcare professionals. - Proactively initiates and case finds to identify and meet all psychosocial, behavioral, and health determinates. - Works on individual clinical projects as assigned. - Demonstrates awareness of hospital and department policies assures compliance with and uses these as a basis for decision making for clinical issues. - The Social Service Case Manager II may also provide supervision to MSW student interns and training of new hires. - Participates in hospital wide performance improvement. - Demonstrates flexibility regarding staffing patterns and resolution of conflicts related to staffing decisions. - Demonstrates awareness of self-learning needs and seeks ways to meet these in order to maintain current competency and respond to new clinical demands. - Develops and maintains effective communication mechanisms with staff, physicians, patients/families and other departments. - Demonstrates independent and effective clinical decision making ability via problem identification/solving skills. - Performs efficiently in emergency situations following established policies and procedures, informing appropriate persons and documenting events. - Non LCSW licensed staff must attend weekly supervision (group or individual). - Attends monthly staff meetings. - Attends Patient rounding/family conferences. - Attends any community meetings as appropriate.
