Inherent in the position is assessment, evaluation, collaboration and coordination of patient care delivery systems and participation in appointed service line activities.  Emphasis is on cost-effective and cost-efficient strategies which maximize quality care within the current health care environment for the identified patient population.  The Case Manager I is responsible for the coordination of utilization management, discharge planning and other health care services/systems. The Case Manager I functions as a contact person, coordinator and facilitator for the patient, family, health care team members, and claims/insurance personnel as necessary.   Major Position Responsibilities: The position characteristics reflect the most important duties, responsibilities and competencies considered necessary to perform the essential functions of the job in a fully competent manner. Qualifications: - Minimum of 3 years clinical and case management experience in acute care setting, preferred. - Knowledge of Prospective Payment System and JCAHO standards and regulatory requirements pertaining to Utilization Management and Discharge Planning desirable. - Ability to communicate effectively utilizing both written and verbal skills. The Diabetes Educator works collaboratively with the management team and physicians to maximize patient outcomes and clinical practice. This individual functions as an educator to patients and an educator, leader, expert practitioner, mentor, and consultant to interdisciplinary staff.  The Diabetes Educator will be actively involved in community outreach to address the diabetes epidemic.  Supports the mission and values of the Medical Center and diabetes care program, and acts as a role model relative to the Medical Center’s Service Excellence Initiative.   Major Position Responsibilities: The position characteristics reflect the most important duties, responsibilities and competencies considered necessary to perform the essential functions of the job in a fully competent manner. The characteristics of the position and standards of performance may be changed by TCMC with or without prior notice based on the needs of the organization. - Maintains a safe, clean working environment, including unit based safety and infection control requirements. - Assesses nutritional needs of patients with includes a diagnosis of Diabetes Mellitus (Type I and II), Gestational Diabetes, Glucose Intolerant, Pre-Diabetes, or as ordered by attending Physician. - Assess nutritional needs of patients with need of glucose control - Provides diabetes education to patients and staff - Educates patients on appropriate care plan for glucose control - Develop educational materials and visual tools for patient education. - Functions as an expert practitioner, mentor and consultant for patients as requested by nurses and other disciplines. - Participates on committees and task forces representing clinical area. - Promotes evidence based practice in the clinical setting. - Participates in multidisciplinary rounds. - Rounds on patients in ARU to aid in home diabetes medication regimens. - Rounds on patients with insulin pumps. - Collaborates with Diabetes Medical Director to maintain compliance with our Joint Commission Disease Specific care certification. - Performs all duties to ensure certification compliance of standards including improving consensus determined metrics. - Participates/Reports at committees/ad hoc projects as determined. - Review/Update any diabetes related policies/procedures. - Serves the community by providing outpatient self-management education to people with diabetes Leads performance improvement processes, recognizing trends, driving changes as appropriate to improve patient outcomes.   Qualifications: - Minimum five (5) years clinical experience, required. - Minimum two (2) years teaching or education experience, required. - Experience with TJC disease specific care certification, preferred. - Demonstrated experience in the application of performance improvement methods, including PDCA, data collection and analysis, and reporting, required. - Understands concepts of reliability, outcomes management and fiscal integration, required. - Experience with CERNER EHR, preferred. - Experience with Microsoft Office Suite (EXCEL, WORD, POWERPOINT, OUTLOOK), required. Education: - Graduate of an accredited school of nursing, OR - Bachelor’s degree form an accredited college or university, providing a curriculum that has been approved by the Commission on Accreditation for Dietetics Education of the American Dietetic Association (ADA), required. - Master’s degree in related field, preferred. Licenses: - Current California RN license, OR - Currently meets all requirements to be recognized as an RD or RDN as per California Code, Business and Professions Code - BPC § 2585. The characteristics of the position and standards of performance may be changed by TCMC with or without prior notice based on the needs of the organization. - Maintains a safe, clean working environment, including unit based safety and infection control requirements. - Ensures that the chemistry department is in compliance with all accrediting and regulatory agency standards, to include but not limited to College of American Pathologists Laboratory Accreditation Program, California Department of Public Health, CLIA ’88 regulations, and Cal-OSHA. - Provides on-site, telephone, or electronic consultation to the chemistry department; maintains effective communication with staff, co-workers, physicians, and other hospital personnel, outside organizations or others who provide or receive laboratory services. - Assists the laboratory director and laboratory leadership in the selection of the test methodology that is appropriate for the clinical use of the test results. - Purchases or directs purchase of laboratory equipment and supplies; evaluates and verifies new instrumentation and procedures; ensures completion of department instrumentation maintenance and service procedures; writes and updates technical procedures and ensures that all technical procedures are reviewed in accordance to accrediting agency guidelines; monitors department inventory appropriately. - Appropriately coordinates proficiency testing in accordance with the department test menu and trained personnel. - Maintains an appropriate laboratory quality assurance program including the development of department quality performance indicators and quality control procedures, for the entire testing process (pre-analytic, analytic, and post-analytic). - Appropriately resolves technical problems and ensures that remedial actions are taken whenever test systems deviate from the laboratory's established performance specifications; ensures that patient test results are not reported until all corrective actions have been taken and the test system is functioning properly. - Participates in interviews of applicants and makes recommendations for hiring; maintains and coordinates an appropriate training and competency program in accordance with regulations of all testing personnel in the chemistry department; provides input to leadership regarding employee performance for possible disciplinary action and for performance appraisals. - Works in production to maintain technical competency in all areas of responsibility, to fill-in during breaks, short-staffing, excess workload, and/ or complex procedures. - Performs other duties as assigned.    Qualifications: - Must have a minimum of four (4) years previous laboratory experience in the subspecialty of chemistry, required. - Minimum five (5) years of experience with increasing responsibility directly related to clinical laboratory, preferred. - Must demonstrate a thorough knowledge and expertise in chemistry, required. - Familiarity with CLIA regulations, quality control evaluation, proficiency testing requirements and safety requirements, preferred. - Ability to operate various clinical laboratory instruments. - Strong written and verbal communications. - Demonstrated leadership qualities. - Possesses written and verbal skills which clearly communicate testing protocols and training for the staff. - Ability to multitask and prioritize assignments. - Must be capable of applying excellent judgment and communication skills to the job. Education: - Meets education requirements for CLS Licensure. Licenses: - Current California Clinical Laboratory Scientist License or Clinical Chemist Scientist License, required. Performs and documents instrument maintenance and procedural calibrations appropriately. - Performs and documents quality control and quality assurance procedures appropriately. - Follows the laboratory's established policies and procedures whenever test systems are not within the laboratory's established acceptable levels of performance. - Appropriately identifies problems that may adversely affect the test performance or the reporting of test results and takes action to correct the problems and communicate with the department technical specialist. - Appropriately documents all corrective actions taken when test systems deviate from the laboratory's established performance specifications. - Follows appropriate infection control practices in the department and wears appropriate PPE. - Performs other duties as assigned. - CLS Trainee: Restricted to the duties of processing and setup of specimens, with no test performance, verification, or release of results. Qualifications: - Must be capable of applying excellent judgment and communication skills to the job. Education:  - Meets education requirements for CLS licensure. Education: - Meets education requirements for CLS licensure. Qualifications: - Minimum of three (3) years’ clinical experience in home health care, required. - Minimum of one (1) year supervisory, educator and/or preceptor experience in home health care, preferred. - Background knowledge and expertise in clinical decision making for the patient population assigned in home health, required.
